Andrew H. Benjamin, JD, PhD and Jeffrey Younggren, PhD of Divisions 31 and 42, and Christina Luini, JD, put together state law specific electronic health record (EHR) templates for each jurisdiction and have made them available at not cost to psychologists. This project was funded by a CODAPAR grant and was completed due to funding and hundreds of volunteer hours.

The EHR templates meet the letter of the law and ethical standards, but also the templates have “drop down menus” that are designed to be elegant. Our EHR Task Force identified the key aspects of good psychological EHRs that physicians and other health care professionals would expect for integrated health care records from their psychologist partners. Also, the EHR templates focus psychologists on meeting the legal requirements of each State’s law– and limit the unnecessary detail that psychologists often include in their records.

The task force supported moving to the World Health Organization’s (WHO) International Classification of Functioning, Disability and Health (ICD) diagnostic codes. And, our free Electronic Health Record templates permit drop down diagnoses using the ICD-10CM functional diagnoses.

These EHR templates should be used as a starting point. As laws change, please contact your state psychological association to verify whether there have been changes in your jurisdiction’s record-keeping duties.
